Financial Policies:
- Patient's portion
of payment is due at the time of service. We will gladly
submit your insurance claim for you; however, we do require any deductibles, co-payments, and "estimated" patient portions
be paid at the time of service.
- We are currently contracted with
the following insurance providers:

Blue Cross/ Blue Shield of Oregon

Cigna PPO
· Healthnet
- We accept cash, checks, Debit
cards, Visa, and MasterCard.
- A no interest payment plan is available which requires a credit approval from an outside agency
and minimal monthly payments.
We offer a 10% cash discount. This applies to payments made in full at the time of service for
payments made in full at the time of service.
Insurance:
· Your insurance plan is designed to share in your dental
care costs. Most insurance plans cover only 50 to 80% of the total dental bill.
· The amount of dental benefits you receive is determined by your employer, your union, or your insurance
company, not by us. The premiums you pay, and the benefits you receive are directly
related and vary significantly from plan to plan. As a general rule, the lower your premiums, the
less benefits you will receive.
· Most dentists' Usual, Customary, and Reasonable fees (UCRs) do not correspond to your insurance
company's standard fees; therefore, we cannot render treatment with the assumption that our fees will be
paid in full by your insurance company.
· Many routine dental services such as radiographs, cleanings, and exams
are not covered by many insurance plans. Even if your plan does cover these services, they
often impose age and frequency limitations such as two cleanings per year or one
exam per year.
· We encourage you to review your insurance policy and understand your specific dental benefits.
We are here to assist you if you have any insurance questions as it relates to your child's treatment.
· In the event your insurance company has not paid their portion within 60 days, the
balance of the bill will become your responsibility. Any overpayment by your insurance company,
if paid directly to us, will be promptly returned to you.
Appointments:
- We work
by appointment only to provide each patient time for proper care. While there is always demand to schedule the most
desirable times such as after school times, we will always do our best to schedule your appointments when they are most convenient.
- Our office hours are Monday thru Thursday:
9 AM to 5 PM.
- We request 24-hour advance notice for cancellations. If less than 24-hour notice is given, a $50.00 cancellation fee per child might be applied to your account.
- We request the parent or guardian accompanying the child to our office also take
responsibilty for payment. We cannot bill a different parent
or guardian who is not involved in the treatment plan.
Fees and
Charges:
- Unpaid balances over 60 days will accrue
a monthly fee equal to 18% APR. Balances over 90 days will be turned over to a collection
agency. In this event,
you will be responsible for all collection and legal fees.
- If a check is returned NSF, there will
be a $25.00 check return charge, and from that point on, checks
will not be accepted.
